Job Description
Provides a comprehensive array of face:to:face supports that engages and enables individuals to be independent and successful within his/her home (own or with family) environment. Encourages the use of natural supports; supports individuals to learn and enhance activities of daily living; and fosters involvement in social (family activities included), and community activities while maintaining safety of the individual. Provides hands:on assistance and instruction with activities of daily living and personal care as required by the individual supported. Assistance with making community and peer connections. The overall goal of supports is to maintain increased independence within an individuals home environment driven by person centered goals and planning. Performs other duties as assigned.

ESSENTIAL JOB FUNCTIONS:

Individual Care
Establishes rapport and working relationship with the individual being supported and their family if they reside within the family home.
Demonstrates sensitivity and commitment to the individuals supported and family members within the home if applicable.
Provides face:to:face hands:on supportive assistance enabling clients to maintain living in their apartment/family home, including activities of daily living, personal care, and other activities which promote healthy living.
Encourages the use of natural supports, and fosters involvement in social (family activities included) and community activities while maintaining safety of the individuals supported.
Supports families and individuals to engage in routine medical, psychological, dental appointments, personal business, and other appointments as needed.
Provides access to community resources through transportation.
Implements Behavior Plan/Guidelines strategies and techniques
Communicates with the Behaviorist on a regular basis to ensure continuity of care is occurring in the home and community.
Provides input in Individual Plan development supporting individuals supported to achieve aspirations and goals.
Attends and participates in identified collaborative team meetings.
Works with community providers for quality care of clients.
Responsible for assigned data input with Therap or designated electronic systems.
Tracks and reports daily data for identified teaching, ADL strategies, and Behavior Plan tracking
Responsible to follow state and Agency guidelines to maintain all appropriate trainings as applicable.
Attends and participates in staff and supervision meetings.
Attends agency sponsored in:services, outside trainings, conferences and meetings to facilitate continued professional growth.


Environmental Responsibilities
Ensure personal vehicle is free of safety hazards/clutter when individual supported is being transported.
Does not contribute to clutter or uncleanliness of family home or an individuals apartment.
Assists individuals with household chores when needed.

Quality Assurance : Chart Documentation and Professional Development
Adheres to policies and practices as outlined in the personnel policy manual, individual program manuals, and as outlined by the quality assurance and safety committees.
Demonstrates competency for population served.
Maintains client records as assigned and in accordance with contractual, licensing, and accreditation requirements.
Participates in agency committee work when available or as permitted.
Schedules, attends and participates in agency meetings, team meetings, and inter:disciplinary team meetings.
Conducts self in a professional manner at meetings, on the job site, and during integration with clients in the community in accordance with agency policies.
Responsible to complete all training requirements as designated by state DDS regulations and as assigned by CoC.
